Trainer Information Form

Purpose of the Tool:
The Trainer Profile Form captures the details of the course trainers and facilitators in terms of their core competencies and experiences in different areas of HIV and AIDS training. The information that Trainer Profile Form collects is expected to help building an data-base of experts in HIV and AIDS training that are available in the Eastern and Southern Africa region.

Usage  
The Trainer Profile Form should be completed by all existing trainers involved in facilitating RATN collaborated training programs at MIs. The Trainer Profile Form should also be completed by all new trainers to help update the database. The information will be entered into a electronic database for records keeping and sharing with RATN
